Flu Shot Drive-Thru Clinics 2020

The Arkansas Department of Health (ADH) will hold drive-thru flu immunization clinics this year to provide the flu vaccine for our school district students and staff.

These drive-thru clinics will be held at the Cabot Health Unit (118 S 1st Street) on the following dates:
 

Tuesday, October 27
2:00 p.m. – 5:30 p.m.

Thursday, October 29
2:00 p.m. – 5:30 p.m.

Friday, October 30
8:00 a.m. – 4:00 p.m.

To receive the flu vaccine, you must:

1. Read the Vaccine Information Statement of the vaccine.

2. Read and complete the front and back of the Arkansas Department of Health (ADH) consent form.

3. PRINT clearly all information required on the ADH consent form.

4. Make sure you have signed the ADH consent form for the flu vaccine.

Please have all paperwork completed before attending the drive-thru clinic.

The flu vaccine will be given to you at no charge. If you have insurance, ADH will ask your insurance company to pay for the cost of giving the vaccine. If you do not have insurance or your insurance does not pay for vaccines, there will still be no charge to you.
