CHS LOFT (Available to high school students only & staff)
8:00 a.m. - 3:00 p.m.

Tuesday, October 15th
Cabot Health Unit (Available to all students & staff)
2:00 p.m. - 6:00p.m.

Thursday, October 17th
Cabot Health Unit (Available to all students & staff)
2:00 p.m. - 6:00p.m.

Flu vaccinations are FREE to all students. If students have insurance, that information will be requested on the immunization forms.

ALL employees must present their insurance card or have their insurance # on the consent form in order to receive the vaccine. Flu vaccinations are FREE to all employees and no co-pay will be charged.

Please fill out the School District consent form and the Health Department consent form.
