Athletic/Band Physicals 2026-27 School Year: Register ONLINE

March 2, 2026

The Cabot Athletic Department, in partnership with Unity Health, will offer Athletic & Band Physicals on April 1st, April 8th, April 10th, & April 15th. 

This physical examination performed by Unity Health will include observation and examination of vital signs, general physical appearance, eyes, ears, nose throat, lymph nodes, the abdomen, skin and the nervous system, heart, blood vessels and lungs.

Registration & Cost

  • All athletes MUST register online before attending
  • Cost: $10 per student (cash or online payment accepted)
  • Proceeds support our Athletic Training Department
  • Register using the QR code (use the grade your student will enter in 2026–27)


All student athletes MUST pre-register online BEFORE attending the physical dates.

* There will be a $10.00 charge this year for every athletic physical given. Money raised will go directly to the district's athletic training department for medical supplies they use on the daily basis to provide the best healthcare to our student-athletes.  You can donate the $10.00 online or in person.

* 3rd period: 11th/12th Softball/Girls XC & Track

* 6th period: 11th/12th Boys Basketball/Swim

Who Must See Their Own Doctor?

  • Incoming 7th graders
  • First-time athletes
  • Students not currently on a CPS roster

These students MUST have a physical completed by their Primary Care Provider on the AAA-required form before tryouts.

If you choose not to attend the physical dates that the school is providing, you may still go to your Primary Care Physician (PCP). Please make sure your physical is for the 2026-2027 school year and is the AAA (Arkansas Activities Association) required form.
